BC Ambulance Service No Cardiopulmonary Resuscitation form:

To ensure that your wishes regarding cardiopulmonary resuscitation (CPR) are legally documented in the community, a No CPR form will need to be filled in.  www2.gov.bc.ca

Palliative Access Line (PAL):
